Alumni Transcript & Certificate Requests

The Registrar can provide additional academic transcripts and certificates of completion at the request of alumni.

There is a $10.00 processing fee for each official transcript or duplicate certificate requested.

Money order and credit card payments are accepted; however, personal checks are not.

To request an official transcript or duplicate certificate simply download the request form below and complete all requested information.

The completed form can be either emailed to registrar@aicm.edu or mailed.

The request is generally honored within 7 business days of receipt.

 
 

Change of Address/Contact Info

If you are a student or graduate who has had a change in contact information (telephone numbers, email or mailing address) since first enrolling in the school, please complete and submit Change of Contact Information Form and we will update.
